Often you do NOT need a referral to come to Physical Therapy. Call our office for more details or to schedule an appointment.
What Insurances are accepted?
We accept most major health insurances, including TRICARE. Please call us to see if we accept your plan.

Where should I park?
There is a parking lot on the north side of the building off of Peach street specifically for Activecare Patients. This parking lot is U shaped for you to conveniently exit back onto Peach street after your appointment. There is handicap parking directly in front of the building.
New patients will find our office staff to be friendly, inviting and you will feel at home the moment you walk through the door or speak with us on the phone.
We encourage you to bring the following items with you on the day of your appointment:
-- Doctor's written prescription for
physical therapy, if you have been given one.
-- Insurance card(s)
-- Photo ID
-- Current list of medications
-- X-ray, MRI or other test results, CD-ROMs
and reports related to the current condition
